My last submission to contest got disqualified.
How/where do I find out the reasons for disqualification ?

You should have gotten an email when it was disqualified that explains what the reason was.
The most common reason is that the algorithm simply can't run - either the backtest or the paper trading threw an exception or crashed. A smaller number of exceptions are because of leverage that is too high.
